Getting Started with Care
Not sure how to organise home care? Just follow these simple steps.
Get in touch with our Registered Manager. We’ll ask for basic information about the person needing care, what type of care you’re interested in, and if you have a start date in mind.
We’ll schedule an appointment for an informal chat over the phone to discuss your needs in more depth.
You’ll receive an email with a summary of our discussion to make sure we’re on the same page.
If you’re interested in proceeding, we’ll confirm a time and date for a formal assessment by email. This will take place at the home of the person receiving care. You’ll need to complete a digital consent to assess form prior to the visit.
We complete the assessment and report the outcome to any relatives who were not present. You’ll receive a breakdown of costings for the level of care discussed.
With written consent, we will then put together a detailed care plan.
Relevant documents will be completed and signed. This includes the legal documents listed below, and forms to help us understand who your loved one is, their likes, dislikes, brief life history and emergency contacts.
We’ll share staff profiles so you can choose your carer
We’ll send a payment confirmation form and contract
Your carer will start visiting at the agreed time.
If at any time your loved one’s care needs or condition changes we can work together to update the care plan. It’s important to us to match carers with people they get on well with. If you or your loved one would prefer to choose another carer at any point please just let us know.
What documents
are involved?
In addition to the care plan, financial agreement and contract, we’ll ask you to provide details about you or the person receiving care to help make sure we’re meeting your needs. This includes what their ideal day looks like, what matters to them, and contact details of next of kin.
We’ll also ask whether the person receiving care has made an advance decision and ask for a written copy, along with copies of the relevant documentation if they have a lasting power of attorney.
